Item Donor-funded project's sustainability assessment : a qualitative case study of a results-based financing pilot in Koulikoro region, Mali(BioMed Central, 2017) Seppey, Mathieu; Ridde, Valery; Toure, Laurence; Coulibaly, AbdourahmaneResults-based financing (RBF) as an alternative to national health financing systems shows a weak level of sustainability due to many factors. In terms of this pilot project in Mali, while some gains could be sustained (investments in long-term resources, high compatibility of values and codes, adapted design to implementation contexts), other intended benefits could not: end of investments; lack of shared cultural artefacts around RBF; loss of different tasks and procedures; the need for more ownership by local stakeholders. High-performing primary healthcare facilities exercised leadership and commitment. These two characteristics were associated with taking initiatives to promote PBF implementation and strengthening team spirit. The PBF project in Mali discussed in this article was funded by the World Bank as part of a larger initiative towards improved reproductive health. Pilot programmes and initiatives have been tested in multiple districts towards improving maternal and child health indicators in the country. The information is provided in brief extracts from studies that relate to health policy and implementation. Emergence of performance-based public policy is lacking and needs to be encouraged further. Problems such as malfunctioning management bodies, or mobilizing staff for meetings prevented adequate implementation of PBF. Communication channels and tools were defective in the vast majority of DHs. In addition, there were problems in motivation of health workers. The article provides background to PBF initiatives and examines specific data in relation to district hospitals.
